Affordable Dental Insurance for Individuals and Families

Individual Choice-Premier is our newest dental program for individuals and families. It's an affordable dental insurance plan that covers 5 levels of benefits ranging from preventive (like cleanings and exams) to major restorative work (and even orthodontics for dependent children under age 19).

With Individual Choice-Premier, you select your dentist from more than 1,350 providers in our Delta Dental Premier network (that's 85% of Oklahoma's dentists) and you can visit more than 118,000 Delta Dental Premier dentists nationwide.

Individual Choice - Premier utilizes a Description of Covered Services table that shows the 5 levels of procedures that are covered (ranging from preventive to restorative and orthodontics for dependents under age 19).

The Description of Covered Services table also shows you the co-payments for each procedure, applicable waiting periods, the deductible for Level 1 through 4 procedures, and the annual coverage maximum for each level of service.

To keep premiums low, there are waiting periods for Level 3, 4, and 5 services. However, from the date you are eligible, you can start saving immediately on Level 1 & 2 services.

No waiting for Level 1 - (Cleanings, exams, x-rays, etc)
No waiting for Level 2 - (Fillings, simple extractions, etc)
12 months for Level 3 - (Select endodontics/periodontics/oral surgery)
24 months for Level 4 - (Crowns, full dentures, etc.)
36 months for Level 5 - (*Orthodontics)
*(For dependent children under age 19. A 36-month waiting period applies)

Annual Maximums:

Level 1, 2, or 3 Annual Deductible is $50:
Level 4 Annual Deductible is $50

Individual Choice - Premier has three monthly rates:
 
Individual Only - $32.38 per month
Individual + 1 (Spouse -or- Dependent) - $59.92 per month
Individual + Family - $113.38 per month (Covers all dependent children under age 19)

IMPORTANT NOTE ABOUT YOUR EFFECTIVE DATE: When valid enrollment documentation and payment is received by DDOK on the 1st through the 20th of the month, coverage will be effective the first of the month immediately following. When valid enrollment documentation and payment is received by DDOK on the 21st through the last day of the month, coverage will be effective the first of the second month. Your first month's premium is deducted immediately upon enrollment. Current Delta Dental subscribers and persons currently covered by another dental carrier are not eligible to enroll.
